Based on the information gathered from the inspection, it was determined that the early learning provider did not submit a written incident report to the department within 24 hours when a serious injury in a child happened at the center. Based on the information gathered from the inspection, it was determined that there was insufficient evidence to suggest the early learning provider reported the incident by telephone within the required timeline to the listed individuals, department and other government agencies when the incident occured. ??
